Sample Submission Forms and Information
The UFPDC is partially supported by the State of Florida as an extension service to the citizens of the state as well as federal grants such as the USDA-NIFA National Plant Diagnostic Network program, substantially offsetting the full cost of the diagnostic process. Sample fees charged to the sample submitter are a co-pay of the full cost of diagnosis. Additional special services requested are charged at cost recovery. Sample fee payment is required at the time services are rendered. We accept credit card payments via our sample submission forms or via a link in your invoice to our online payment portal. These fees directly support the students, staff, and supplies required to perform the diagnostic service. Please select the appropriate diagnostic form for your sample type and indicate payment on the form or pay online upon receipt of the invoice to ensure timely release of the final diagnostic report.

Submission Forms
International Sample Submission
The PDC-GNV accepts and diagnoses samples from outside of the United States. The diagnostic fee is $250 for international general samples and $300 for international turf samples. Those interested in our international service must first email Dr. Carrie Harmon, clharmon@ufl.edu, to obtain forms, permits, and pertinent collection information.
Hemp Sample Submission
The UF-IFAS Plant Diagnostic Center in Gainesville, FL can receive industrial hemp samples for diagnosis. Samples must be accompanied by a current FDACS commercial license and a completed sample submission form. Please email the lab with any questions: pdc@ifas.ufl.edu
